STEPS TO SUBMIT YOUR CREDIT APPLICATION TO MILAM'S TRUCK AND TRAILER SALES:
(Please do only ONE of the following.)

Click Here to open the Credit Application and then fill it out.

1. Microsoft Outlook
   When you are finished filling out the Credit Application, click on the "Click Here To Send Your Form By Email" button. This will automatically take you to your Outlook client and from there you can click send and have it submitted directly to us.
2. Web-Based E-Mail (Hotmail, Yahoo!, Etc.)
   When you are finished filling out the Credit Application, save a copy to your computer. Then go to your e-mail and start a new e-mail message and attach the saved Credit Application and send the e-mail to milamtrk@gamewood.net.
3. Fax
   When you are finished filling out the Credit Application, click Print. Then Fax it to us at (434) 822-5693.
